Skilled Nursing facilities (also known as Nursing Homes) are designed for seniors who need 24-hour care. These homes usually offer many of the same services as other senior living facilities, such as meals, personal care, and supervision, as well as different forms of therapy. What makes Skilled Nursing facilities different from other senior living options is the presence of on-site medical staff at all hours.
- Basic Care includes the services needed to keep up daily living activities, such as personal care, supervision, and safety.
- Skilled Care includes services for treatments and procedures by a registered nurse, as well as specially trained professionals, such as physical therapists.
- Sub-Acute includes comprehensive in-patient care for those suffering from acute illness, injury, or chronic illness.

Families Loving Their Choice

What a breathe of fresh air knowing that this company provides such a personal touch. They treat their clients with respect and compassion. They take into consideration the details that other companies so easily leave out. It’s a shame more companies aren’t like them, especially working with such fragile situations. They are the best of the best in my eyes. I had the chance to meet the owner and she embodies trust, love and passion for finding the best living/care situation for her clients. Angela is the type of business owner that would literally give you all she has to find what you need. We look forward to a continued relationship with you guys! Cheers!

My wife and I love my mother-in-law but we didn’t have the assisted living expertise to be able to evaluate what’s best for her. Angela (our counselor from LYC) drove out twice and talked to mom. Angela used her terrific interpersonal skills combined with her medical expertise to determine exactly what’s right for mom. Literally dozens of factors were taken into account to find the right place. What impressed me the most is LYC took a comprehensive approach. Both emotional and physical well-being needed to be fully satisfied. Angela showed us a couple places and we eventually found the perfect community for mom. To be brutally honest, it’s not only worked out great for mom but also for us.

Thank heaven we found Living Your Choice!
Recently, our 90-year-old mother became ill and wasn’t going to be able to live on her own anymore, as she’d been doing. My sisters and I were trying to find an assisted living arrangement for her in San Diego, and we were quickly overwhelmed by the options. (It’s very difficult to find a place on your own, just going by the info you see online. Many places don’t post their rates, so you have no idea if you’re looking at something within your budget or not.) I put out a plea on Facebook for any local friends who could recommend a good assisted living place, and a friend suggested we contact Living Your Choice for help.

I wanted to take a moment to thank Living Your Choice for helping me with my dad Bill.
Thank god I had heard of Living Your Choice through a mutual friend. I called and found an expert staff that was able to help me right away. We spent over an hour on the phone discussing my dad, his life, his likes and dislikes, his medical issues, his budget and what he was going to need moving forward. Right away I felt so much better, they understood what was going through and agreed to meet the next morning at the hospital with us both.

Assisted living provides seniors with personalized care, social opportunities, and a safe, supportive environment that enhances independence and quality of life.
Skilled nursing care provides 24-hour medical attention and rehabilitation services for individuals with complex health conditions or recovering from surgery. These facilities employ licensed healthcare professionals who administer treatments, monitor progress, and provide daily assistance with personal care. Skilled nursing is designed to support recovery and improve overall health in a medically supervised setting.
Skilled nursing facilities offer a range of services including post-surgical care, medication management, physical and occupational therapy, and assistance with activities of daily living. These facilities are equipped with medical technology and staffed by professionals trained in acute care. The goal is to provide a safe, rehabilitative environment that supports recovery and helps residents regain functional independence.
Individuals needing skilled nursing care typically have medical conditions that require round-the-clock supervision, rehabilitation, or recovery assistance following surgery or illness. These residents may require professional support for medication management, wound care, and mobility assistance. Skilled nursing care is ideal for those whose health needs exceed what can be managed in less intensive settings like assisted living.
Medical care in skilled nursing facilities is coordinated through comprehensive assessments, personalized care plans, and a multidisciplinary team approach. Physicians, nurses, therapists, and other specialists collaborate to ensure that each resident’s health is monitored closely and that interventions are implemented promptly. This integrated approach ensures continuity of care and rapid response to any changes in a resident’s condition.
Rehabilitation services in skilled nursing facilities often include physical therapy, occupational therapy, and speech therapy. These services are tailored to help residents regain mobility, strength, and independence after surgery or illness. A dedicated rehabilitation team works with residents on personalized goals, using structured exercise regimens and therapeutic techniques to support recovery and enhance quality of life.
Skilled nursing facilities offer comprehensive support for daily living activities including bathing, dressing, eating, and mobility. Trained staff assist residents with these tasks while encouraging as much independence as possible. Personalized care plans ensure that each resident receives the level of assistance necessary to maintain dignity and comfort while promoting overall health and rehabilitation goals.
Insurance coverage for skilled nursing care varies and may include private insurance, Medicare, or Medicaid. Many facilities have dedicated staff who help residents and families navigate the billing process and understand coverage options. It’s important to verify insurance details and coverage limits beforehand to ensure that the chosen facility meets both the medical and financial needs of the resident.
Quality of care in skilled nursing facilities is monitored through regular inspections, accreditation standards, and continuous performance evaluations. Resident feedback, health outcomes, and staff training programs are used to ensure that high standards are maintained. This ongoing oversight helps identify areas for improvement, ensuring that residents receive the best possible care in a safe and supportive environment.
Family members are encouraged to be involved in the care process by participating in care plan meetings, visiting regularly, and communicating openly with the medical team. This collaborative approach ensures that the resident’s needs are clearly understood and met, and it provides families with the reassurance that their loved one is receiving attentive and compassionate care.
During the transition, staff will conduct a thorough assessment to create a personalized care plan, and a dedicated team will help with the move-in process. You can expect detailed orientation regarding daily routines, available services, and care procedures. The facility aims to provide a smooth transition that minimizes stress while ensuring immediate support for the resident’s medical and personal needs.
